Testosterone: low, in range or high?

Relative to your laboratory’s reference range — not a personalised TRT target.

LOW

Below the laboratory range

Sexual wellbeing: Lower libido or fewer morning erections may occur.

Daily activity: Fatigue or reduced strength may occur; other causes are common.

What it does not prove: One low result does not establish a diagnosis or dose change.

Low result: next step

Next step: discuss symptoms and sample timing with your clinician. Untreated low results often need repeat morning testing; during TRT, follow preparation-specific instructions.

IN RANGE

Within the laboratory range

Sexual wellbeing: Symptoms still matter; a normal result does not explain every concern.

Daily activity: No guaranteed energy level, gym performance or recovery speed.

What it does not prove: It does not confirm that TRT is effective or free of side effects.

In-range result: next step

Next step: review symptom changes and blood count with your clinician; keep testing at the agreed interval.

HIGH

Above the laboratory range

Sexual wellbeing: Higher is not automatically better for libido or erections.

Possible concerns on TRT: Acne, mood changes or raised haematocrit need review; they can also occur with in-range testosterone.

What it does not prove: A high reading may reflect dose timing or gel contamination.

High result: next step

Next step: ask your prescriber to review the result, dose timing, possible contamination and blood count. Do not change your dose yourself.

What is TRT?

TRT stands for testosterone replacement therapy. It is a medical treatment used for appropriately assessed patients with testosterone deficiency, usually involving prescribed testosterone injections or gel.

The aim is to improve symptoms associated with deficiency while achieving appropriate testosterone levels and monitoring for adverse effects.

A blood test can help assess testosterone levels, but treatment decisions also depend on symptoms, medical history and clinical assessment. TRT is not suitable for everyone with tiredness, low mood or difficulty building muscle.

Already using TRT and arranging your blood tests privately?

You can attend MDC for testing while continuing treatment with your existing prescribing clinician.

Our service is designed for patients who want professional blood collection, a clear record of their results and a consistent clinic for repeat appointments.

You may be:

Starting prescribed TRT and establishing your monitoring routine.
Checking levels after a clinician changes your dose or preparation.
Comparing current results with earlier blood tests.
Investigating why symptoms have not improved as expected.
Arranging blood tests requested by a private or online prescribing clinic.
Looking for regular, in-person testing close to work or home.

Bring your previous results and your prescriber’s requested test list where available.

Taking testosterone without medical supervision? You can still contact us about testing and clinical assessment. Blood tests alone cannot establish that unsupervised treatment is safe.

Single Testosterone Blood Test
Total testosterone
MLTES

A focused measurement of total testosterone.

Suitable when your clinician has requested a total testosterone measurement or when this forms part of an agreed investigation.

Your sample is collected from a vein at our Baker Street clinic.

A single testosterone result does not provide a complete TRT monitoring assessment. Your clinician may also need blood-count, prostate or other tests.

Testosterone Profile
Testosterone + SHBG + Albumin + Free testosterone
MLTESPR

A fuller picture of testosterone and the proteins that affect its interpretation.

The profile includes:

  • Total testosterone.
  • Sex hormone-binding globulin, or SHBG.
  • Albumin.
  • Free testosterone, with the reporting method explained on your laboratory report.

This can help your clinician interpret a total testosterone result in context.

The Testosterone Profile does not automatically include a full blood count or PSA. Additional monitoring tests may be needed.

TRT Progress Monitoring Programme

A planned series of clinic appointments, paid for together.

For patients who need repeated testing, our programme provides a consistent place to attend, collect blood samples and compare results over time.

Choose an agreed schedule of monthly or two-monthly appointments where appropriate to your treatment plan.

Before you enrol, we confirm:

  • The number of appointments.
  • The tests included at each visit.
  • Any additional tests requested by your clinician.
  • The timing of samples in relation to your medication.
  • How results will be reviewed and shared.
  • The total programme price.

Your agreed programme is paid for as one package. Any investigations outside that agreed scope are explained and priced before they are arranged.

TRT before and after — how do you know whether treatment is working?

A useful TRT before-and-after comparison combines blood results, symptoms and clinical review.

A higher testosterone number alone does not prove that treatment has worked well for you.

Your clinician may consider:

What to compareWhy it matters
Testosterone resultsShows the hormone concentration achieved with your treatment
SHBG and free testosteroneHelps interpret total testosterone in context
Haemoglobin and haematocritChecks for an excessive rise in red blood cells
SymptomsAssesses changes in sexual desire, energy, wellbeing and other symptoms relevant to your diagnosis
Side effectsHelps identify whether treatment needs reassessment
Dose and sampling timeMakes comparisons between visits more meaningful

Bring any blood results taken before you started TRT. If these are unavailable, current testing can still support your ongoing care, but it cannot recreate your untreated baseline.

TRT transformation — look beyond photographs

Online TRT transformation photographs often focus on changes in muscle or body shape. They cannot show whether a person’s treatment is appropriate, whether their blood count has risen or whether their testosterone level is suitable.

Physical changes can also reflect training, diet, weight loss, lighting and other factors.

At MDC, we focus on measurable blood results that can be considered alongside your symptoms and clinical progress.

Successful treatment is individual. It should not be judged against another person’s photographs or an expectation that the highest testosterone result is the best result.

Why choose in-clinic TRT blood testing at MDC?

Professional venous blood collection

Your sample is collected by a trained member of our team in a private clinical setting. You do not need to collect a finger-prick sample yourself or arrange its return by post.

Attention to medication timing

The timing of a sample relative to an injection or gel can change how the result is interpreted. We record your medication details and sampling time and follow the agreed collection instructions.

A consistent clinic for repeat appointments

Return to our Baker Street clinic for your planned blood tests. Keeping a record of your previous results, treatment changes and sampling times helps your clinician assess trends.

Testing that matches your monitoring needs

You can book a focused testosterone measurement or arrange a broader monitoring programme. We explain what is included so you can match your booking to your clinician’s request.

A clear next step

Your results should support a treatment discussion. We explain report delivery and the available review arrangements before you book. Results requiring prompt attention follow our clinical escalation process.

How often should I have a TRT test?

The right interval depends on how recently treatment started, the preparation used, dose changes, previous results and your medical history.

Some patients need closer checks during treatment adjustment. Others can move to less frequent monitoring once treatment is stable.

MDC can arrange monthly or two-monthly appointments when appropriate. These are options for an agreed monitoring plan, not a requirement for every patient.

Your clinician should determine which tests need repeating at each visit. It is not always necessary to repeat every investigation each time.

What is TRT post-treatment testing?

“TRT post testing” can mean two different things:

  • Testing after starting TRT or changing a dose: this checks your response while receiving treatment. The appointment must be timed appropriately for your preparation.
  • Testing after stopping TRT: this assesses hormone levels after treatment has ended. The timing and investigations depend on the medicine used, when it was stopped and your symptoms. LH, FSH or other tests may be needed alongside testosterone.

A single post-treatment result cannot confirm that your natural testosterone production or fertility has fully recovered.

Tell us which situation applies before booking. Do not stop prescribed testosterone simply to obtain a blood test unless your clinician advises it.

How your MDC appointment works

1

Choose your test or discuss a programme

Book an existing testosterone test or contact us about a series of monitoring appointments.

2

Confirm preparation and timing

Provide your requested test list, testosterone preparation and dose. Follow your clinician’s instructions about when to attend relative to your medication.

3

Attend our Baker Street clinic

Every appointment involves an in-person venous blood collection at MDC.

4

Receive and compare your results

Keep your reports together and share them with the clinician managing your TRT. For programme patients, review arrangements are agreed before enrolment.

5

Plan your next appointment

Arrange further testing according to your treatment plan and findings, rather than automatically changing your dose in response to a number.

Your TRT questions answered

Can I book without moving my TRT prescription to MDC?
Yes. You can arrange blood testing at MDC while continuing treatment with your existing prescriber.
Can blood tests tell me whether TRT worked?
They can show how hormone levels and relevant monitoring markers have changed. Whether treatment has worked also depends on symptom improvement, side effects and clinical assessment.
Is a higher testosterone result always better?
No. A higher result is not automatically a better result. Interpretation depends on your preparation, sampling time, symptoms and the treatment goals agreed with your clinician.
Do I need to test in the morning?
For investigating untreated low testosterone, morning testing is often important. During TRT, the timing relative to your specific injection or gel is also important. Follow the instructions agreed for your treatment.
Should I take my testosterone before the appointment?
Follow your prescriber’s instructions. Do not skip, delay or change a dose without advice. Tell the sampling team when you last used your medication.
I use testosterone gel. Does that matter?
Yes. Gel can contaminate a blood sample and produce a misleadingly high testosterone result. Tell us before collection so the appropriate precautions can be followed.
Does the programme include medication or dose adjustments?
The programme is for agreed blood testing and review arrangements. Medication, prescribing and dose adjustments are not automatically included. These remain the responsibility of your treating clinician unless separately arranged.
Can I monitor fertility through these blood tests?
These tests do not measure sperm production. Testosterone treatment can suppress fertility, so tell your clinician if you are trying to conceive or planning a family. Separate fertility assessment or semen analysis may be appropriate.
Is home testing always less accurate?
Not necessarily. A validated home test can provide reliable results when the sample is collected and handled correctly. MDC offers professional venous collection in a clinic, with staff managing the collection process and preparation checks.
What happens if my treatment stops during a prepaid programme?
Contact us before your next appointment. Your testing needs may change, and the programme terms will explain how remaining appointments, changes or refunds are handled.
