Marylebone Diagnostic Centre Membership Policy

 

  1. Introduction

This policy outlines the terms and conditions governing the Marylebone Diagnostic Centre Membership Plan. It is designed to protect the interests of both the members and the Centre while ensuring a professional and high-quality service. By enrolling in the membership plan, members agree to comply with the terms set forth in this policy. 

  1. Membership Eligibility
  • Membership is open to individuals aged 18 and above. 
  • Family Membership covers two adults and one child under the age of 18. Additional family members may be added at an extra cost. 
  • All applicants must provide accurate and complete information during the registration process. 
  1. Membership Activation and Duration
  • Membership becomes active upon receipt of the first payment. 
  • The membership is valid for a period of 12 months from the date of activation. 
  • Members will receive a renewal notice 30 days before the expiration of the membership. 
  1. Membership Benefits
  • Members are entitled to the services outlined in their specific membership plan. 
  • Services included in the membership plan must be utilised within the membership period and cannot be carried over to the next period. 
  • Any additional services outside the scope of the membership will be charged at the standard rate. 
  1. Payment and Refunds
  • Membership fees are due at the time of enrolment and may be paid annually or monthly. 
  • All payments are non-refundable, except in cases where services are not provided due to the fault of the Centre. 
  • The Centre reserves the right to adjust membership fees annually, with prior notice given to members. 
  1. Cancellation and Termination
  • Members may cancel their membership within 14 days of activation for a full refund, provided no services have been used. 
  • After the initial 14-day period, membership fees are non-refundable. 
  • The Centre reserves the right to terminate membership in cases of non-compliance with the policy, misuse of services, or any behaviour that disrupts the professional environment of the Centre. 
  1. Service Usage and Appointments
  • Members are required to book appointments in advance to avail of the services included in their membership. 
  • Missed appointments without prior notice may result in forfeiture of that service for the period. 
  • The Centre will make every effort to accommodate appointment requests, but availability may vary based on demand. 
  1. Data Privacy and Confidentiality
  • The Centre is committed to protecting the privacy and confidentiality of all member information, including health records. 
  • Personal data will be processed in accordance with applicable data protection laws and will only be used for the purposes of providing and managing membership services. 
  • Members have the right to access their personal data held by the Centre and may request corrections or deletions in accordance with the law. 
  1. Changes to the Membership Plan
  • The Centre reserves the right to modify or discontinue any part of the membership plan, including the services offered, with 30 days’ notice to members. 
  • Any changes will be communicated to members via email or through the Centre’s official website. 
  1. Member Conduct
  • Members are expected to conduct themselves in a respectful and professional manner at all times while on the Centre’s premises. 
  • Any form of abusive or inappropriate behaviour towards staff or other members will result in immediate termination of membership without refund. 
  • Members are responsible for adhering to the Centre’s health and safety guidelines during visits. 
  1. Limitation of Liability
  • The Centre will not be held liable for any indirect, incidental, or consequential damages arising from the use of membership services. 
  • The Centre’s liability in any case is limited to the amount of membership fees paid by the member. 
  1. Dispute Resolution
  • Any disputes arising from the membership plan will be resolved amicably through discussions between the member and the Centre’s management. 
  • If a resolution cannot be reached, the dispute will be subject to mediation.
